In clinical trials, the occurrence of somnolence has been reported in some patients (6 of 853 patients) taking azelastine hydrochloride and fluticasone propionate nasal spray [see Adverse Reactions]. Patients should be cautioned against engaging in hazardous occupations requiring complete mental alertness and motor coordination such as operating machinery or driving a motor vehicle after administration of azelastine hydrochloride and fluticasone propionate nasal spray. Concurrent use of azelastine hydrochloride and fluticasone propionate nasal spray with alcohol or other central nervous system depressants should be avoided because additional reductions in alertness and additional impairment of central nervous system performance may occur [see Drug Interactions].

Numerous studies comparing psyllium to control animals have noted a quantitative increase in fecal weight and moisture content independent of changes in food intake, due mostly to increased water resorption in the colon. [21] This has been demonstrated in humans assessed by fecal analysis following oral consumption of psyllium husk [65] [66] with one study noting two out of four subjects experiencing a doubling of fecal weight. [22] In a rat model of Colectomy (Colon removed, the moisture content of feces in this rat model at 65-70% better mimicks human moisture content at 70-75%; normal rats reach 12-55% [8] ), ingestion of either whole psyllium or the gel forming fragment can increase moisture content from an expected 65-70% to 86-90%. [8]

Patients requiring oral corticosteroids should be weaned slowly from systemic corticosteroid use after transferring to Advair Diskus. Prednisone reduction can be accomplished by reducing the daily prednisone dose by mg on a weekly basis during therapy with Advair Diskus. Lung function (mean forced expiratory volume in 1 second [FEV 1 ] or morning peak expiratory flow [AM PEF]), beta-agonist use, and asthma symptoms should be carefully monitored during withdrawal of oral corticosteroids. In addition, patients should be observed for signs and symptoms of adrenal insufficiency, such as fatigue, lassitude, weakness, nausea and vomiting, and hypotension.

Azelastine hydrochloride displayed no sensitising potential in the guinea pig. Azelastine demonstrated no genotoxic potential in a battery of in vitro and in vivo tests, nor any carcinogenic potential in rats or mice. In male and female rats, azelastine at oral doses greater than 3 mg/kg/day caused a dose-related decrease in the fertility index; no substance-related alterations were found in the reproductive organs of males or females during chronic toxicity studies, however, embryotoxic and teratogenic effects in rats, mice and rabbits occurred only at maternal toxic doses (for example, skeletal malformations were observed in rats and mice at doses of mg/kg/day).
